Who We Are
Ability Home Care, LLC is a licensed, insured, state-regulated, and Medicaid-certified company located in Woodstock, Virginia that provides in-home and long-term care services. Since 2006, we have been offering Person-Centered In-Home Care services such as case management, personal care, and respite care services through our compassionate and dedicated attendants. With our services, we help our clients remain independent in their own homes and communities.

Our staff will always remain honest and respectful in all the services they provide. They will respect the individual’s right to privacy, including their personal belongings, and treat each person with dignity.

The team will educate our patients and their families on how to provide assistance with managing the patient’s illness, disease, or disability. This helps us promote independence in daily activities.
Consistency
Ability Home Care shall strive to maintain consistency in staff placements. All attendants shall follow the client’s plan of care and ensure each client receives the same quality of care at each visit. We will also make it a priority to be punctual and to be accurate in documenting all activities.
Honesty
All our attendants shall be honest with all clients. All information in the patient’s plan of care is strictly confidential and will not be shared with non-staff.
Integrity
None of our attendants shall take advantage of the client in any way. We will always strive to fulfill all responsibilities and tasks outlined in each patient’s plan of care. In addition, no employee shall solicit clientele for private care.

Education
We strive to provide education and raise awareness among our clients regarding disease processes and medication management. Ability Home Care maintains a high level of competency by requiring all staff members to complete the attendant certification training program before their first assignment. In addition, our attendants must undergo 12 hours of ongoing training annually.

CCC+ Waiver Program Eligibility
We cater to individuals that are covered by the Elderly & Disabled Commonwealth Coordinated Care Plus (CCC+) Waiver Program. CCC+ is a Managed Care Program which is a fully funded for individuals who are Medicaid Eligible or Dually enrolled Medicare/Medicaid Eligible. This program is targeted for individuals who are either 65 or older, children and adults with disabilities, nursing facility residents, and those receiving services and supports through the home and community based waiver. Individuals who have conditions that require ongoing medical care are eligible for this waiver program. Examples of medical conditions include, but are not limited to, Parkinson’s disease, dementia, stroke, congestive heart failure, and diabetes.
